Nursing Reflection

Going back through previous experiences and tracing back the footsteps we have made allows us to reminisce about the good things that have happened in our lives. Somehow the undesirable memories would also seep in as they are part and parcel of our existence that we cannot do away with. This process gives us an opportunity to encounter past events that can necessarily aid us in the future. In the Nursing practice, reflection is a retrospective approach that evaluates historical processing of experiences that takes place in a structured form and is deemed highly essential (Eliis, Kenworthy and Gates, 2003, 156). In the clinical practice, this retrospective activity facilitates in the promotion of quality care. The art of reflection however in the nursing practice focus on self rather than on the situation as the care provider (Quinn, 2000, 252). The process is a reflective practice that is a cognitive act by which we are allowed to make sense of our thoughts and memories (Taylor, 2000, 43). This method therefore allows a practitioner to generate a complementary or alternative form of knowledge and a set of choices in the evaluation of the best course of action. It is a â€Å"deep learning† experience that reflects on our knowledge and theories and go beyond merely thinking about what we do but involves recalling what had occurred and analyzing the situation by interpreting important information recalled (Taylor, 2000, 4). In Nursing, the reflective process is aimed about our own practice (Taylor, 20000, 3); that nursing education and research cannot do without as a common practice in the learning mechanism in which we all engage in a regular basis (Slevin and Basford, 2000, 483). With a main purpose of enabling the practitioner to learn from experiences and increase clinical effectiveness, reflection is highly essential to the nursing practice. For this process to be effective, Johns has provided a guided reflection which employs different models of self-inquiry to enable a practitioner to realize desirable and effective practice (2002:3). Considering that this involves a cognitive and emotional component that is expressed through analysis, different models would aid us feel comfortable about the activity. John’s model can be used in preparation for or during clinical supervision and applicable to specific incidents rather than more general day to day issues and particularly applicable to those who prefer a structure approach (Ellis, Kenworthy and Gates, 155). Gibbs Model use term description rather than â€Å"a return to the entire experience† as a form of reflection is considered as a simpler method but one where a mentor or facilitator is likely needed(Davies, Bullman and Finlay, 2000, 84). Both models however in supervision practice can be used to facilitate clinical governance through the promotion of quality care where an exchange between two professionals employing this technique seeks to improve their practice (Watkins, Edwards and Gastrell, 2003, 266). To maximize the potential benefits of clinical supervision, nurses have to learn to be comfortable with this retrospective activity with the aid of Gibbs or John’s models depending on where one feels most comfortable working with (Ellis, Kenworthy and Gates, 156). Gibbs Model for reflection As a simple and easily attainable method, Gibbs model uses term description rather than a return to a previous experience (Davies, Bullman and Finlay, 84). In psychology and teaching, reflection facilitates as purposeful change and competencies such as psychological-mindedness and self-regulation (Clutterback and Lane, 2004, 196). Usually this process involves a mentor, teacher or supervisor working with a student at different stage while allowing for individuality. Although less specific than re-evaluating an experience; Gibbs in his cycle or reflection makes the action planning a more overt component of reflection (Davies, Bullman and Finlay, 84). Gibbs provides that in one’s own practice, an essential aspect of working as an autonomous practitioner involves a critical analysis of one’s role and responsibilities from a personal perspective (Gibbs, 1998,13). It is a process that requires others to become involved that encourages feedback and constructive comment to recognize your role and value in a health team (Humphris and Masterson, 2000, 77). John’s Model for reflection John’s model uses the concept of guided reflection to describe a structure supportive approach that helps the practitioner learn from their reflections and experiences (Quinn, 2000, 572). The approach involves the use of a model of structure reflection, one-on-one group supervision and the keeping of a reflective diary (Quinn, 572). The practice would aid the practitioner in learning from a reflection of their experiences. John’s model is more detailed as it provides a checklist of specific points necessary for reflection (Davies, Bullman and Finlay, 85). The only problem cited with John’s model if it imposes on a framework that is external to the practitioner leaving little scope for inclusion as cite by other theories. John’s model can be used in preparation and during clinical supervision consisting of 6 steps that is applicable t specific incidents rather than more generalized day to day issues facing the supervisee (Ellis, Kenworthy and Gates, 155). This model is highly attractive to those who prefer a structured approach but others may find this type more restricting (Ellis, Kenworthy and Gates, 156). Criticisms against the reflective process Reflection involves cognitive and emotional components that are expressed through analysis and to maximize the potential benefits of the clinical supervisor nurses have to learn to feel comfortable with this retrospective activity both during and in preparation for supervision sessions (Ellis, Kenworth and Gates, 157). This could be deemed time–consuming in an institution where time is often an important element in the delivery of care. A time for reflection can be done positively only when a situation or a need arises. This is probably why reflection method is considered a radical approach to nursing education and practice given the ample time training can afford (Slevin and Basford, 483). Yet reflection is valuable if done in partnership with someone else which led Davies et al to believe that the approach is quasi-therapeutic (Davies, Bullman and Finlay, 86). The principles have been transferred directly from client-centered psychotherapy and may trigger more powerful responses such as guilt and anxiety. Practitioners are therefore evaluated before they are given a chance to try this one out according to conservative studies. However with practice, it is assumed that a reflective process may not hold as much negative impact for the learned practitioner in an answer to the demands for a continuous review of a practice in a critical and analytical manner that support the reflective concept. The Value of Reflection for the Student Nurse As an essential component of scholarly practice, reflection, reflection is a method for generating a complementary alternative form of knowledge and theory (Humphris and Masterson, 2000:78).   Regardless of any negative criticism a reflective method may elicit from critics, I consider this to be a valuable tool. For the student, this is a process were one internally examines and explores an issue of concern triggered by an experience that clarifies the meaning of perspectives (Canham and Bennett, 2001, 185). The nursing practice has been surrounded by a world of silence and reflection is a way for nurses to reflect that is enhanced and introduced in the nursing curriculum (Guzzetta, 1998, 102). Often in the professional practice, nurses have encouraged silence among themselves in their health environment and setting while usually developing a shared professional voice with her team. Oftentimes, her relationship with the rest of the health team and other professionals faced difficult efforts because of the autonomy. The process of reflection allows one to air out her sentiments and ideas within her group or to a mentor or a supervisor during moments of reflection that could be produced as a shared voice for the team. Developing a habit of reflection is therefore a must for nursing education in order to uncover dimensions of experiences such as hidden and explicit meanings of behavior that can aid a student nurse in identifying her own perspective of the nursing practice that is highly useful in her entry to the profession (Guzzetta, 1998, 103). For a student in nursing, one must therefore develop a habit of reflection in order to uncover experiences and the meaning of behavior, values and thoughts that could readily prepare one for professional practice. It should be noted that the reflective process can helpfully aid in teamwork where one has the chance to relay sentiments after reflection of her past experience.Nursing education must therefore develop and evaluate innovative strategies to prepare nurses to meet the challenges of the rapidly changing health care system and for lifelong learning (Johns and Freshwater, 1998, 149). Reflection and reflective practice are currently receiving attention as a strategy yet little is known about the process of becoming a reflective thinker, how to teach skills needed for reflection, or the barriers and facilitators to becoming a reflective practitioner (Clutterback and Lane, 2004, 198). However a reflection process is worthy of study and practice that should initially be started and adapted as a core training for everyone wishing to professionally practice nursing as a positive way to analyze the development of reflective practice abilities. A Literary Analysis of Still Alice Essay

Still Alice (Genova, 2009) is a captivating debut novel about a 50-year-old woman’s sudden decline into early onset Alzheimer’s disease. The book is written by first time author Lisa Genova, who holds a PH.D in neuroscience from Harvard University. She’s also an online columnist for the national Alzheimer’s association. Her other books include Left Neglected and Love Anthony. She lives with her husband and two children in Cape Cod. The theme of the book is related to the early onset Alzheimer’s disease and how the main character, Alice Howland’s quality of life or in other words her lifestyle is affected due to the disease she is diagnosed with (Genova, 2009). The novel sheds light on the lives of those struggling with this horrendous disease of the mind and how their lives and people in them are affected and disturbed because of it. The life and daily activities of an early on-set Alzheimer’s disease patient worsens with time, if prop er counseling sessions and treatment are not conducted; their situation will get worse and result in severe damage to their mental health. As the story begins, everyday quarrels reside in the Howland household. From their youngest daughter Lydia’s ongoing dispute about her future, to Alice and John’s own relationship, all while their busy lives ensue. Why couldn’t Lydia be like the rest of her family? Her brother Tom and sister Anna followed in their parent’s footsteps. Going to college and having successful careers was their way of life. John is a biologist and Alice a professor of linguistics, both of them work at Harvard University. Lydia is the outcast of the family. She travels the world, is worry free, and aspires to be an actress. The hectic lives of both Alice and John weigh on their relationship. Tension increases with the story as Alice is diagnosed with early onset Alzheimer’s disease. The coping skills of Alice and her family with her diagnosis set the tone for the story. Alice’s internal conflict consumes her, although she tries to never let Alzheimer’s disease define her. She is upset with herself for forgetting things, yet trie s to come up with reasons as to why she is forgetting. Her father takes the brunt of all Alice’s anger. Her hatred for him grows. Why did he have to take the lives of her mother and sister? Is it his fault Alice has Alzheimer’s? Alice continues with a strong desire to contribute to society. The once estranged relationship between Alice and her daughter Lydia gradually improves, and a  daughter finds solace in a mother she never knew. A husband, who at one time stood by his wife through everything, surprises his family and leaves his ailing wife behind for a new job opportunity. Still Alice is told in the third person point of view with limited omniscience. The narrator is Alice Howland, the main character of the story. Through the book we are able to read the way Alice is feeling, looking through her eyes as the story unfolds. However, in the final chapter of this novel the point of view changes to her husband, John Howland. Readers are able to look on as John lives his life away from Alice in New York. The primary settin g for still Alice (Genova, 2009) is in Cambridge Massachusetts were the main character Dr. Alice Howland lives with her husband, John Howland, and teaches cognitive psychology at Harvard University. Other settings include their home in Chatham Massachusetts where Alice and John go to vacation. The main character is Dr. Alice Howland a modern middle aged professional woman. She’s a brilliant professor experiencing mental glitches like forgetting a word while giving a lecture, misplacing keys, etc. Alice thinks this is due to impending menopause, middle age or possibly stress. But she hasn’t gone through menopause and she’s not feeling stressed. The defining moment is when Alice forgets her way home while jogging in Harvard square (p. 21). Suspecting that something is wrong and without telling her husband, Alice chooses to see Dr. Davis a neurologist who diagnoses her with early onset Alzheimer’s disease (p. 70). The diagnosis hits Alice and her family like a death sentence. Alice realizes that she doesn’t have enough time left and decides that it should be spent with her family. Her two older children Anna and Tom decide to undergo genetic testing for the Alzheimer’s gene. Her oldest daughter Anna tests positive for the gene (p. 108). Alice becomes completely reliant on her blackberry to get through her day and becomes a great list maker though she can’t always make sense of her lists. As the disease progresses Alice resigns her teaching position at Harvard University and gives up jogging because she has lost her sense of depth perception. Although the disease robs Alice of her memories, she retains her sense of humor e.g. when she tries to wear her underwear as a sports bra, â€Å"she laughs at her own absurd madness† (P. 199). Alice’s character is very compelling, engaging, and holds your attention throughout the book. John Howland is Alice’s husband; he is a professor of biochemistry at Harvard University.  When John finds out about Alice’s diagnosis, he thinks that she has been misdiagnosed. He insists on genetic testing, which would show that Alice has the Alzheimer’s gene. John struggles to deal with Alice’s diagnosis. He becomes her primary caretaker, but refuses to look at Alice when she takes out her pill box. He manages to continue working by leaving Alice large reminder notes on the refrigerator not to go running alone (p. 196). John is obviously grieving the loss of his wife but is unable to express his emotions in a positive way. A breakthrough finally happens when John is able to comfort his wife when she experiences an incontinence episode (p. 150). Anna is Alice’s oldest daughter; she’s a successful lawyer and is married to Charles, also a lawyer. Anna is strong and fiercely independent just like her mother. Anna deals with her mother’s disease by suggesting that if her mom â€Å"thinks for a second† then maybe she’ll be able to remember things (p. 173). Anna, however still makes time to care for her mom when her dad is away. Alice’s only son Tom is a minor character in the book. He is present during family gatherings and important moments e.g. when Alice discloses that she has Alzheimer’s disease (p. 103). He deals with his mom‘s diagnosis by being distant. The main antagonist in the book is Alice’s youngest daughter Lydia Howland. She defies the family’s educational tradition and chooses to take acting classes in Los Angeles in hopes of becoming an actor. Lydia’s decision causes a lot of conflict between her parents. She’s the only one that refuses to participate in the Alzheimer’s genetic testing. However, as Alice’s condition deteriorates, Lydia becomes the most valuable player of the family; she becomes Alice’s number one advocate. She doesn’t try to â€Å"test† Alice’s memory but is there for her when she needs her (p. 162). The relationship between Alice and Lydia improves. Lydia eventually listens to her mother’s advice and enrolls at Brandies University to study theater (p. 258). The imagery in Still Alice is captivating. It keeps the reader on their toes, wondering what will happen next in the story. The story is so moving that the reader can essentially picture the characters and endure their experiences through all five senses. One model of imagery that is intriguing is when Alice is a guest speaker at Stanford. Her confidence and love for her career is enticing. She had talked about the particular subject numerous times and every time she had done this, she felt respected. Alice had always talked without reading her  notes with ease. Well into her discussion she suddenly becomes puzzled. â€Å"The data reveal that irregular verbs require access to the mental†¦Ã¢â‚¬  (p. 10). The word was lost even though she understood fully what she had intended to say. As the scene is described, the reader can get a sense of panic that Alice is experiencing (pp. 8-11). An unforgettable illustration of imagery is when Alice goes for a run through Harvard Square, blocks from her home and cannot remember her way back. The description of her surroundings is etched in the reader’s mind. â€Å"Steady stream of joggers, dogs and their owners, walkers, rollerbladers, cyclists, and women pushing babies in jogger strollers, like an experienced driver on a regularly traveled stretch of road† (pp. 20-21). â€Å"The sounds of her Nikes hitting t he pavement in syncopated rhythm with the pace of her breath† (p. 20). â€Å"She knew she was in Harvard Square, but she didn’t know which way was home† (p.21). â€Å"People, cars, buses, and all kinds of unbearable noise rushed and wove past her† (p. 22). The reader can sense Alice’s state of fright during this scene. A memorable instance of imagery is when Alice visits her primary care physician. At one point and time Dr. Moyer’s office held no negative links for Alice. â€Å"I don’t think your memory problems are due to menopause† (p.41). â€Å"She fought the impulses growing louder inside her, begging her to either lie down or get the hell out of that examining room immediately.† The reader can perceive the state of anxiety Alice is in (p.41). Other doctor visits were just as interesting. â€Å"Alzheimer’s disease†¦.The words knocked the wind out of her†¦The sound of her name penetrated her every cell and seemed to scatter her molecules beyond the boundaries of her own skin† (pp. 70-71). The visit to Mount Auburn Manor Nursing Center was striking. â€Å"Walkers crowded the spaces between the tables†¦There was no socializing, no conversation†¦The only sounds other than eating came from a woman who sang while she ate† (p. 113). The reader gets the impression that Alice feels she doesn’t belong there. Another prominent illustration of imagery is when Alice gets lost in her own home. â€Å"She hastened back down the hall and opened the door to the bathroom†¦Only, to her utter disbelief it wasn’t the bathroom† (p. 149). The reader understands that Alice’s disease is progressing, especially when she doesn’t recognize her own daughter or her own family. â€Å"Leave me alone! Get out of my house! I hate you! I don’t want you here† (p. 211). Alice couldn’t understand that it was her family having a dispute about her, nor did she understand why. â€Å"She  wasn’t sad or angry or defeated or scared†¦she was hungry† (p. 264). The most remarkable moment in the novel that captures the audience is when Carole returns Alice to her home after their walk. As Carole and Anna converse about Alice, Alice smiles and nods her head as to concur, not knowing that they are speaking of her. Shortly after, Lydia and Anna converse about their father coming to visit. â€Å"Is Dad coming this weekend? asked the act ress† (p. 291) The reader comprehends that John has decided to take the new job offer, instead of fulfilling Alice’s wish for them to stay together. There are many symbols within the story of Alice Howland: objects, actions, or characters that hint a meaning beyond themselves. Clocks play a role throughout the entire book. For Alice clocks are guidelines to what she should be doing at each moment of the day. In the start of her story, the clocks are more of a reference, but as her disease progresses they are much more than that. Alice becomes dependent on the clocks to tell her the things she is supposed to do according to time. Alice’s Blackberry device is probably one of the most significant symbols in Still Alice. The Blackberry becomes a part of her as Alzheimer’s takes over; Alice refers to her Blackberry for everything. To Alice, if something was not recorded in her device, it never existed. Within the Blackberry, she had an alert asking her five questions every day. These five questions became a reflection of her cognitive level, testing herself to see if her disease has progressed. The questions she quizzes herself include a P.S. â€Å"if you have trouble answering any of these, go to the file named â€Å"Butterfly† on your computer and follow the instructions there immediately† (p.119). The file named â€Å"Butterfly† signifies dignity to Alice; the file instructs her to commit suicide when she can no longer remember her family. This would have become the last thing she could do for herself. Alice also has a butterfly necklace that had been passed down from her mother. This butterfly necklace seems to calm Alice and to give her hope, clarity, and strength. Genova’s use of figures of speech brings the reader into the world of a person with Alzheimer’s disease. When Alice first suspects that there is something wrong with her, she is referred to a specialist for testing. Having completed various tests, Alice begins to feel overwhelmed, comparing her thoughts to a roller coaster that she was riding with her eyes shut, unable to know when the next turn would be (p.40). Later, in the same doctor’s office, Alice learns of her official  diagnosis of early-onset Alzheimer’s disease and is stunned. With the use of language, Alice and the r eader are brought back to the harsh reality of losing herself. â€Å"The sound of her name penetrated her every cell and seemed to scatter her molecules beyond the boundaries of her own skin (p.71). Genova relates Alzheimer’s disease to a beast that is unable to be slayed, giving the reader the knowledge that this disease cannot be beat. In reference to the medications Alice is taking to try to prevent the further decline in her mental state she compares them to aiming squirt guns at a blazing fire (p. 117). Throughout the book, Alice feels embarrassed and like a burden to those around her. The use of personification of her disease tells the reader exactly how embarrassed she is â€Å"there it was, her Alzheimer’s, stripped and naked under the fluorescent lighting† (p.129). It is further conveyed to the reader when Alice thinks her husband looks at her the same way he looks at his labs rats (p. 135). In a later scene when Alice refuses to go to dinner with friends the use of simile brings the disease to colorful life in Alice’s world. â€Å"I’m a cotton candy pink elephant in the room. I make everyone uncomfortable. I turn dinner into a crazy circus act, everyone juggling their nervous pity and forced smiles with their cocktail glasses, forks, and knives† (p.218). To keep the book from not being completely negative, there is humor to Alice’s Alzheimer’s symptoms. When she was unable to recall which door in her house lead to the bathroom, Genova’s use of humor is exactly what the reader needs to get through the scenes. â€Å"She swung the door open like an illusionist revealing her most mystifying trick, but the bathroom didn’t magically appear† (p.150). Nearing the end of the novel, Genova leaves the reader with one last insight into Alice’s cognitive decline. In a moment of clarity Alice reads the cover of the book she wrote with her husband â€Å"The words she read seemed to push past the choking weeds and sludge in her mind to a place that was pristine and still intact, hanging on† (p. 284). The story of Sti ll Alice is based on an ironic situation. Dr. Alice Howland is a psychology professor at Harvard University and an expert in linguistics (the study of language and its structure). She finds herself at a crossroads when she is diagnosed with Early Onset Alzheimer’s Disease at the age of fifty. Dr. Howland also keeps remarkable care of her well-being by eating healthy, running Harvard square each day, keeping her mind active between teaching classes, and attending  conferences. While talking over her new diagnosis Alice comes to realize she may have misjudged her father, assuming he was a belligerent alcoholic, in reality she realizes he was likely suffering from undiagnosed Alzheimer’s disease (p.76). To top off the ironic chain of events Alice receives her diagnosis of Early Onset Alzheimer’s on January 19 (p. 67) which happens to be the date of her mother and sister’s deaths. Towards the end of Alice Howland’s story, her disease process has taken its toll on Alice’s mind and she c an no longer care for herself independently. Dr. Howland becomes dependent on her daughters and son, whom she can no longer identify as such, for her care while her husband choses to take a job in New York (p.286-292). In conclusion, Still Alice is the story of a happily married professor with three grown children at the peak of her career who is diagnosed with early onset Alzheimer’s disease. As the story unravels, Alice struggles to maintain her lifestyle, independence, and to live in the moment. The book not only brings to light the devastating effects of Alzheimer’s disease on its victims, but also on the lives of their friends, families, and relationships. Tension increases within the story from the time Alice is diagnosed with early onset Alzheimer’s disease until her husband leaves. It was heartbreaking that Alice’s husband chose to abandon his beloved wife for a new job opportunity in another city. He failed to stay by her side as this disease robbed her of herself. Still Alice is a strong and engaging fiction work which brings truth to the average reader about the onset and the impact of Alzheimer’s. However, it fails to address those families who are not as equipped as the Howland’s to care for their loved ones at home, and are forced to institutionalize them. As Alice’s condition deteriorates, her children begin to worry about their futures, fight amongst themselves, and ultimately change their lives as a result of their mother’s disease. This story teaches the valuable lesson that no matter how strong relationship or family ties are, the failing health of a love one could have a devastating impact on everyone.

Two Drovers and William Wilson

â€Å"William Willson† and â€Å"The Two Drovers† suggest a link between identity and aggression. What threatens a characters sense of self in these stories? Why do certain characters react to perceived threats to their identities as they do? â€Å"The Two Drovers† by Sir Walter Scott focuses on an Englishman and a Scotsman in a tale revolving around nationality. The author characterizes them both as proud individuals, while also noting their individual talents and temperaments, but the most prominent trait in both main characters is pride for their own countries. Hence both are presented as national stereotypes, and it is from this that the author is able to build upon and highlight the growing misunderstanding which is the true essence of the story. It is however, their pecuniary and vocational interests that allow them to find the common ground on which they base a mutual respect for each other. For example, when English and Scottish cattlemen are droving livestock together â€Å"†¦ hey co-operate on the journey with one man as guide and interpreter in the Highlands, and the other in England, Robin Oig and Harry Wakefield form a partnership of mutual advantage. † , when this partnership falls prey to what could almost be described as a comedy of errors, they are then forced to revert to their respective social values and customs to fatally resolve what is essentially a commonplace misunderstanding. These differences, propelled by the force of pride, culminate in a true tragedy. Harry Wakefield, with his short fuse and strong fist and Robin Oig, with his Scottish pride and secret ambitions both possess too much pride to back down from a heated situation, and ultimately die as a result of a simple misunderstanding. In the final battle scene he writes he â€Å"felt within his single arm the energy and power of a multitude†. He feels this because within him are his will, his conscience, and every other part of his soul. There is a need to develop social cohesion between generations and within the older generation to understand the matter in correct perspective. An effort has been made in this paper to focus on this key issue and highlight the importance of inter and intragenerational relationships to quality of life to older people. The present demographic profile of the United Kingdom reflects a number of consistent and marked trends. People are living increasingly longer, the birth rate is showing a steady decline and the mean age is increasing steadily. By 2031, the over 80s population is predicted to double to 5 million, according to the Office of National Statistics. If the health of the elderly population follows current trends that will mean people spend more of their lives fighting various illnesses. Since 1981, the amount of time women can expect to live in poor health has increased by 15%. For men, the length of poor health rose by 34% (Triggle, 2004). Besides health, older people face the mobility problems, poor transport and lack of finance. Car usage declines with age and varies by sex. 77 per cent of men and 64 per cent of women aged 65-74 in 2001 in Great Britain had access to automobiles (National Statistics, 2006). Today, we can observe a significant change in family structures. The realignment of family ethos has caused varying degrees of separation between the young and old members of families and affected familial relationships negatively. As people age and their working lives end, they may have more free time available to spend in a social circle. Barriers of older life further enhance the necessity to promote inter and intragenerational relationships. This paper scrutinizes the various dimensions of quality of life at old age. It overviews the factors affecting quality of life and highlight the significance of relationships at old age. Components of Quality of Life  Quality of life is a wide ranging term. The Department for Work and Pensions of UK (2006) identifies five domains as the most relevant to assessing progress in older people’s quality of life: 1) Independence within inclusive communities. 2) Healthy, active living. 3) Fairness in work and later life. 4) Material well-being. 5) Support and care. Factors Affecting Quality of Life of Older People Good quality of life in old age is a realistic ambition for all. Early old age, sometimes called the Third Age, between 55 and 75 years old, is the phase of life between leaving the labour market and the onset of physical dependency. There are a number of factors that affect quality of life at this age. A research study carried out by Dr Blane (2003) concluded that quality of life for the affluent-healthy is higher than for the deprived-sick. An intragenerational perspective identifies similar age groups and the succession of individuals who are distinguished by the bounds of a society and set apart by time and common experience (Fredriksen, 1993). Historically, within the UK, there had been a strong tradition of the young learning from the old in their role as respected elders or through structures such as apprenticeship schemes and trade guilds (Hatton, 2002). Significance of intergenerational relationships is evident from a survey which reports that to the elderly, welfare of children and grandchildren is twice as more important than their own health and money (Roy, 1999). Older parents have always been providing financial support in the forms of donations, gifts and regular monetary contributions to their children. They are also important sources of instrumental support (shopping, cleaning, home maintenance, childcare) and emotional support (advice, validation) for their children (Bengtson, Rosenthal Burton, 1990). Many elderly people continue to provide support for adult children and the provision of a home for one’s children can extend well beyond middle age (Evandrou, Falkingham, Rake Scott, 2001). Living arrangements are a key dimension of quality of life and well-being in old age. Older people living alone are more likely to enter an institution than those living with other people (Breeze, Slogett Fletcher, 1999). Involvement of older persons in local communities contributes to their quality of life, as well as to the functioning of the community. The relationship between different groups of older persons is therefore very crucial in maintaining the quality of life (UN-Economic and Social Council, 2002). Altered patterns of mortality and fertility have made intergenerational structures in the family more â€Å"top heavy† and vertically extended. While horizontal, intragenerational ties are shrinking, vertical ties across generations are more complex and durable than ever before in history (UN-Department of Economic and Social Affairs, 2005). Importance of social groups and networks in later part of life is quite evident from a research study carried out in Scotland (Philip, Gilbert, Mauthner and Phimister, 2003). The participant observations and questionnaires have provided useful insights into the levels and types of participation in social groups among older people in the study area. The report finds that there is high level of participation and involvement in social groups amongst older people (aged over 60). As a whole, respondents attended 68 wide-ranging types of groups including civic participation (e. g. local government), voluntary activities (e. g. Red Cross; Meals on Wheels), friendship, social, leisure, religious, cultural, heritage and special interest groups. Those aged in their seventies and early eighties were the most likely to be involved in social activities Another study summarises that older people preferred the company of others, had a higher level of contact with friends and lower levels of psychological problems (McKee, 2002). In 2002 around three in ten men aged 80 and over and nearly one in five women in England said they owned a mobile phone. Use of mobile phones and the Internet helps older people to remain independent by making it easier for them to communicate with their family and friends or to access public and commercial services (National Statistics, 2006). Conclusion Society in the UK is changing. More people are living longer in greater prosperity. In the next decade, the numbers over 50, over 65 and over 80 will all increase to levels never seen before. We need a coherent strategy to manage that demographic change. Elaborate and diligent plans are required to enable older persons to continue to participate fully in all aspects of life. For a better quality of life, we need to promote the development of a society for all ages through the strengthening of intergenerational and intragenerational solidarity. INTRODUCTION Communication is the process of exchanging ideas, facts and figures from one person to another. The term cross cultural implies interaction with people of different cultural, ethnic, age and class backgrounds. Communication is interactive in nature and is greatly influenced by our relationship with others. Different words may mean different meanings to different people. In addition to this what makes cross cultural communication really difficult is the difference in language even the same word can mean different things. Cross cultural communication is a form of global communication which describes how people of different culture communicate and interact with each other in different ways among themselves. Cross cultural communication is a field of study that draws various ways that we have learned to speak and gives non-verbal messages. Thus miscommunication is likely to happen when it involves significant cultural differences among the communicators. This is what known as Cross Cultural difference. FUNDAMENTAL PATTERNS OF CULTURAL DIFFERENCES There are six different cultural patterns that tend to vary from one person to another person. These are discussed as follows: Varying communication styles The way people communicate varies widely within people of different cultures. What may mean to one person may have a different meaning to other person. This aspect of communication style is known as language usage. Some words carries different meaning across different cultures. Another aspect of the communication style is the degree of importance given to verbal and non-verbal communication. Non-verbal communication not only includes gestures and facial expressions but also involves sense of time, personal distance etc. Different approaches to completing the tasks Approaches to completing tasks vary from culture to culture. The reason behind this is different judgments of the rewards associated with the accomplishment of the task, different notions of time and access to resources available. For example Asian culture tends to give more importance to the developing relationships and emphasize more on the task completion towards the end as compared to the European-American culture. Varying attitudes towards conflicts Some cultures see conflict in positive manner while others view it as something to be avoided. In the U.S conflict is not considered to be desirable however people often are encouraged to directly face conflicts as they arise. In eastern countries conflicts are considered to be embarrassing; a written exchange might be favorable means to address the conflict instead of speaking. Different styles of decision making The role played by an individual in decision making process varies from culture to culture. For example in US decisions are frequently delegated i.e. an official assigns the responsibilities to the subordinates associated with the task while in Japan consensus is the preferred mode. Different attitudes towards disclosure In some cultures it is not considered appropriate to be open about emotions, personal feelings and information or a conflict or misunderstanding. While working being early or on time for all appointments, meetings whether formal or informal is a show of respect and sincerity towards the task while any event of being late requires an apology. Communication styles Understanding communication style is another biggest challenge that is often faced while working in Japanese culture. Japanese have a preference for indirect and high context communication. They emphasize more on the impact of body language, developing relationship, emotions and other non-verbal communication. Harmony In Japan there is more emphasizing on collectivism rather than individualism. They place significant emphasize on loyalty towards the group. In Japanese culture it is important to understand that praising or prioritizing any one individual over others is likely to be embarrassing. Excessive hard working attitude Japanese people are highly dedicated to their work. For them, there is no day-night, hunger-thirst. When they are determined to achieve something, they achieve it any cost without thinking about any kind of comfort. OVERCOMING CROSS CULTURAL DIFFERENCES No matter whom you are or where you live or which culture you come from it is important to recognize that although other people may dress like you or even work for the same company they are not just like you. It is very helpful in checking out the meaning of what someone says by repeating back what you have heard which in turn helps you to confirm what has been said accurately. Food packaging, as an example, economically utilizes resources that if prepared in another manner would result in massive waste. The processing and packaging of food permits the residues to be utilized as either feed for animals, food by products or fuel. The FTC states (Vitamin Lawyer, 2005): â€Å" Advertising claims based solely on traditional use should be presented carefully to avoid the implication that the product has been scientifically evaluated for efficacy.† The purpose of the Food and Drug Administration is (U.S. Food and Drug Administration, 2005): â€Å"†¦ protecting the public health by assuring the safety, efficacy, and security of human and Veterinary drugs, biological products, medical devices, our nation’s food supply, cosmetics, and products that emit radiation.† This broad mandate stretches far beyond the actual words used to comprise the agency’s title, Food and Drug Administration. The FDA’s mission statement goes on to add that is also assumes responsibility to advance the health of citizens in the United States by helping to â€Å"†¦ speed innovations that make medicines and foods more effective†¦Ã¢â‚¬  (U.S. Food and Drug Administration, 2005), as well as â€Å"†¦ helping the public to get the accurate, science based information they need†¦Ã¢â‚¬  with respect to the utilization of medicines along with foods for the purpose of improving their health. A genetically modified organism is one where the generic material has been changed utilizing techniques that are generally known as recombinant DNA technology (wikipedia, 2005). The labeling practice that the U.S. Food and Drug Administration found to be misleading is that these companies were giving the misleading impression that their foo d products were safer than products made with GMO ingredients. Consumer Faced with a plethora of purchasing possibilities in all manner of products, from food items to electronics, vehicles, clothing, toys, furniture and other areas, consumers are barraged with all manner of subtle advertising and copy tricks to sway them from one brand to another. Faced with these realities, companies employ subtle copy tricks to appeal to consumer sensibilities, as even a small difference can make a favorable increase in register sales. That image of a real strawberry on your favorite cereal package is not what is contained in the box, nor is the bread that states it is made from real wheat flour which is fortified with â€Å"†¦ vitamins and minerals †¦ that build strong bodies in 12 ways†¦Ã‚ ¨(Crawford, 2004). In fact, the additional information contained on labels as a result of governmental regulation might actually be helping companies to promote their hype as few consumers actually read this information which is a part of everything they buy (Crawf ord, 2004). Knowing yourself and what you can do, can help you recognize and overcome your weaknesses.   Ã‚  Ã‚  Ã‚  Ã‚  One of my greatest strengths at work that I have recognized would have to be my ability to be a well-organized individual. I tend to write what I have to do down on paper and prioritize what is more important to least important. In doing this, it helps me organize and accomplish my work and meet deadlines that are important. A personal strength that I have at home is patience. I am a mother of a beautiful three year old little girl who is a handful. Patience is my greatest strength as a parent and individual which I possess especially being a single mother. You may be wondering what there interests have to do with my being a doctor and why I have chosen this career path rather than that of a paramedic, a police officer, or even a news reporter. That is easy. As far back as I can remember, I have been fascinated by anything having to do with doctors. It is for all my interests in the medical field that I have decided to become a doctor. The field of medicine that would allow me to combine all these fascinations is forensic pathology. As a forensic pathologist, I will be working firsthand with investigators and the hospitals in cases where patients have died of trauma or unknown causes. Investigators will have to share all the details of the crime scenes or accidents with me. Then, I will get to play the role of Nancy Drew by putting all the pieces together to determine the actual cause of death. I will try to find out exactly what happened to every one of my patients in their last minutes of life. I have always been intrigued by mysteries; therefore, I feel this career as a forensic pathologist will be perfect for me. It is important to enjoy one’s career, so this is an excellent choice for me because it will never become dull or boring. Each autopsy that I will perform will have its own story and a new mystery for me to solve. I know I will wake up every morning anxious to go to work to see what new findings I will discover that day. You probably are wondering how I can be so excited about p eople’s dying, but I do not look at it like that.
